Ahh... react to how he comes out, take the good option and score. Solved.
All of the instances that I have seen of penalty shots being stopped are related to the attackers ballsing it up by hesitating/being indecisive. Often it does not matter what you do, as long as you do it well - you will score.
Jordan, in your first example... you flicked it into the defenders legs. OK... where was your other attacker? As soon as the puck hits his legs the other player should be there to put it in.
Regarding flicking the puck into a static defender... as a ref I would say that if the player is able to move his legs out of the way and does not then it is a foul and the unbeliever should be punished.
